## Form fields

- `payment_network_id`: radio button or hidden field, required when more than one payment network is available.
- `account_id`: text, required. Account Number
- `account_id_confirmation`: text, required. Confirm the account number before requesting a cash code.
- `phone_number`: text, optional. Used for verification and optional SMS delivery.
- `phone_region`: select, optional. Country/region used to validate the phone number.
- `email`: email, optional. Used for verification and optional email delivery.
- `send_email`: checkbox, optional. Send cash code via email.
- `send_sms`: checkbox, optional. Send cash code via SMS.
- `sms_opt_in`: checkbox, required before SMS verification or SMS delivery.
